Central High in Cooperation with the Chickasaw Nation will Provide a Free Flu Shot Clinic

Flu season is quickly approaching. Central High in cooperation with the Chickasaw Nation Public Health nursing team will provide free flu shots to our students, family and employees at Central High. We have scheduled the nurses to arrive on Friday, October 29, 2021 from 9 am – Noon to provide flu shots to those wanting to participate. The event is free and open to anyone regardless of race. See below attached the consent form that will need to be completed and signed by the parents/guardians as well as anyone wanting a flu shot. The insurance area may be left blank. There is a place for the parent/guardian to check that their child may receive the flu shot without them being present. However, we welcome any parent/guardian that would like to be present when their child gets their vaccination.
